Review your progress with an experienced diabetes educator…just as you would in a dietitian’s office, without the hassle of appointments and office waits. Online consultations include a comprehensive review of your current diabetic medications, current blood sugar levels vs. target blood sugar levels, diet understanding & compliance, and most recent lab work (if available). The dietitian will make recommendations for improvement based on the information you provide and modify your diabetes plan as needed to achieve blood sugar control and minimize you risk of diabetes complications.
Consultation includes 1 initial online message from you in addition to your personal details form (to be completed before consult), a return message with recommendation from the dietitian and a final follow-up by email or phone to assess understanding.
